The employee's official job title. Examples are "Assistant Pricipal," "English Teacher," "Custodian II," and "Food Service Worker." |
Salary Adjustment Profile |
If this employee is typically part of a group that shares a common set of contractual terms, select his/her group profile from the drop-down menu.
It is also okay to leave this blank if the employee does not fit into a defined group. |
Days in School Year |
The number of work days in the school year. It is usually 260 for public schools but may be 261 in some years. |
Location |
The location in which the employee will work at the school. Examples are "Glenora High School," "Buildings & Grounds," and "Eastview Middle School Cafeteria." |
Signature Deadline |
If you will be sending the employee a notice of wage/salary, enter the date on which his/her approval of the wage/salary is due. |
Applicable Date Range |
Enter the dates between which the salary/wage applies. It is a text field, so you can enter dates in any format you desire. For example "July 1, 2014 to June 30, 2015" or "07/01/2014 to 06/30/2015." |
Pay Variable Group |
If you selected a group profile from the Salary Adjustment Profile drop-down menu, and that group has an associated Pay Variable Group, select the Pay Variable Group from the drop-down menu.
If you did not select a Salary Adjustment Profile, or if there is no Pay Variable Group associated with the group profile you selected, leave this field blank. |
Standard Annual Raise |
This is the increase in wage or salary that the employee will receive:
Standard Annual Raise = New Base Salary/Wage - Old Base Salary/Wage
Examples:
- If an employee received $14.50/hour previously and will now receive $15.00, you will enter .50 in the Standard Annual Raise field because $15.00 - $14.50 = $.50
- If an employee received $54,750.00 in salary previously and will now receive $55,000.00, you will enter 250.00 in the Standard Annual Raise field because $55,000.00 - $54,750 = $250.00.

Additional Annual Raise |
If the employee should receive any other monies not included in the standard raise, longevity bonus, merit increase, etc., enter that amount here.
Leave blank if not applicable. Typically this field is left blank because stipends, attendance, longevity, grad hours, in-service, etc. are handled elsewhere.

Longevity Bonus |
If the employee is entitled to a stipend for longevity enter the full amount. For example .30 or 1.20. Omit dollar signs ($). |
Merit Increase |
If the employee is entitled to a stipend for merit enter the full amount. For example 1000.00 or 150.00. Omit dollar signs ($). |
New Salary/Hourly Rate |
Enter the employee's new salary or hourly rate. For example:
- If an employee received $14.50/hour previously and will now receive $15.00, you will enter 15.00
- If an employee received $54,750.00 in salary previously and will now receive $55,000.00, you will enter 55,000.00

If the employee is entitled to a stipend for achieving perfect attendance enter the full amount. For example .32 or 1.10. Omit dollar signs ($). |
Budget Code |
The employee's budget code (sometimes called the "org"). Leave blank if not applicable. |
Budget Object |
The employee's object code. Leave blank if not applicable. |
Budget Project |
The employee's project code. Leave blank if not applicable. |
Request Pay Selection |
Check the checkbox if the employee is required to select the number of pay periods he/she receives in a year (i.e. 22 vs 26 pays). Leave the checkbox un-checked if the employee is not given this choice. |
Current Pay Selection |
This is the number of pays the employee is currently receiving annually (e.g. 22 or 26). |
Hours Per Day |
The number of hours the employee works per day. If the employee is salaried or if the hours per day are irrelevant, you can leave this blank. |
Employee Comments |
Leave blank. This will be filled in if the employee challenges his/her notice of wage/salary. |
